先日blogでも紹介した「iPhone破損、カード保険申請」の件、今週無事に保険金を受け取る事ができました。
Compared to the past, the time between submitting the documents and receiving them may have been longer.
今回の場合は、こちらのApple正規サービスプロバイダーさんでiPhoneの「交換」を選択。交換および手数料込み18,700円のうち免責分3,000円を差し引いた15,700円を保険会社より振込にて受領しました。
The documents that were required for the application procedure are as follows.
- 保険金請求書
- パスポートコピー（日本出入国のスタンプのページ及びお写真のページ）
- 警察官公庁・航空会社・旅行添乗員・第三者による事故証明書本紙
- 自認書（第三者照明をお持ちでない場合）
- 修理費用の見積書（本紙）または領収書（本紙）
- 損害物件購入時の領収書・クレジットカード利用明細書・保険証書等の本紙
- 損害物件のお写真（全体のお写真と破損箇所のお写真）
In my case, I submitted a letter of self-recognition, and the content of my self-recognition was quoted verbatim from the notes I took there.
« ‘At around 19:00, 20120511, while walking near Barcelona Passeig de Gracia station, he made contact with a pedestrian and dropped the iPhone in his hand.’
This memo was written in advance in anticipation of the submission of the self-identification form. This is the third time I’ve applied for card insurance, so I’m used to it (laughs).

盗難があってから2時間後にskypeを通してカード会社に連絡。保険会社のコールセンターの電話番号を教えてもらい、直後にすぐ連絡。現地警察で盗難証明的な書類があるとよいと言われたが、滞在日程やロシア語学力を考慮すると警察へ行き被害届を出す時間はない、また一人であるため第三者による証明も難しいという旨を伝えたところ、自認書のみでも申請可能といわれたため。
